Visits and fees
A 15-minute video visit is available to discuss your questions, booked and paid online in advance. It is not required to choose a program. If a visit needs more time, Dr. King asks before extending it. Dr. King does not respond to clinical questions, prescription requests, or scheduling inquiries outside a paid visit. Established patients use the patient portal for care that is already underway. Fees for any further care are given to you in writing before it is scheduled.
How billing works
My practice runs on a retainer. After your first 15-minute visit, care begins with a retainer of $2,500 kept on account. Visits, my time between visits, labs, test kits, supplements and prescriptions ordered through the practice are all charged against it at my regular prices.
My time outside visits is billed in 7.5-minute units, and every portal message I read is billed at least one unit. When your retainer runs out, only medically urgent care continues until a new retainer is in place. You receive an itemized statement each month, and any unused balance is refunded when care ends.
I do not bill insurance. All communication goes through the patient portal; text messages are not answered.
Cancellation terms are shown when you book and in your confirmation email.
Prepaid Package Terms
Prepaid multi-session ketamine packages are prepaid, non-refundable, and non-transferable. The reduced per-session price is the consideration for committing up front, so a package cannot be refunded once purchased and sessions cannot be given to another person. A patient who changes their mind, moves away, or stops coming is not refunded.
Prepaid sessions do not expire. There is no time limit within which they must be used.
There are three circumstances where money comes back. First, if Dr. King discontinues your treatment, whether for clinical or safety reasons or because she is unable to deliver the remaining sessions, every unused session is refunded in full. Second, if a patient dies, unused sessions are refunded to the estate on a pro-rata basis. Third, if a patient becomes medically unable to continue because of a condition lasting more than thirty days and confirmed by a licensed physician, the patient may either pause the package at no cost for the duration and resume afterward, or cancel and receive a pro-rata refund of unused sessions.
Committing is a choice, not a requirement. Anyone who prefers not to prepay can pay per session at the full rate. The Ketamine Intake is separate from every package and is not refundable.
These terms are provided in writing and acknowledged before any package is charged.
Your right to a Good Faith Estimate
You have the right to receive a Good Faith Estimate explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
